Quick answer
Forma skin tightening in Markham at Dr. Monique Mazzuca Aesthetics starts at $350 per session and is performed personally by a licensed physician (MD, CCFP) at 200 Cachet Woods Court, Suite 309, in a 60-minute appointment. Forma by InMode uses radiofrequency energy to gently heat the deeper layers of the skin, without needles, to stimulate collagen and improve firmness and elasticity on the face and neck. It feels like a warm massage, needs no numbing and has no downtime beyond brief redness. Results are gradual and subtle, building over a series of sessions and kept up with periodic maintenance. New patients start with the $50 Comprehensive Aesthetic Consultation.
What Forma is
Forma® by InMode is a non-invasive radiofrequency (RF) skin tightening treatment. A smooth handpiece is moved over the skin on a layer of gel, delivering radiofrequency energy that warms the deeper layers of the skin (the dermis) while the surface stays comfortable. There are no needles, no incisions and nothing is injected.
The handpiece is designed to monitor skin temperature during treatment and regulate the energy it delivers, so the tissue is held in a therapeutic warm range rather than overheated. That is what makes Forma a gentle, no-downtime treatment, and also why its results are subtler than those of treatments that break the skin, such as Morpheus8.
How Forma works
Collagen is the main structural protein of the skin. From early adulthood we gradually make less of it, and the collagen we have becomes looser and less organized, which shows up as softening contours, fine lines and a loss of bounce. When the dermis is warmed to a controlled temperature, two things happen. Existing collagen fibres contract, which can give a slight immediate firming. More importantly, the heat stimulates fibroblasts, the cells that produce collagen and elastin, to lay down new fibres over the following weeks.
Because each session delivers a modest, controlled amount of heat, the effect is cumulative. A single treatment gives a warm glow; a series is what produces a meaningful change in firmness. Forma works on the quality and tightness of the skin itself. It does not replace lost volume and it does not relax the muscles that cause expression lines.
Forma in Markham: what it treats
- Mild skin laxity on the cheeks, along the jawline and under the chin, including early softening of the jawline.
- Neck rejuvenation: mild looseness and crepiness of the skin on the neck.
- Elasticity and firmness: skin that has lost its bounce, often first noticed in the mid-thirties to fifties.
- Fine lines and overall facial rejuvenation, with the forehead and around the eyes treated using appropriate settings where Dr. Mazzuca considers it suitable.
- Maintenance between or after more intensive treatments such as Morpheus8, and a no-downtime refresh before an event.
Forma is best for mild changes and for patients who want to stay ahead of them. It will not lift heavy jowls or significant neck sagging, which are surgical concerns, and it does not fill hollows, which is the role of dermal filler or Sculptra and Radiesse. Dr. Mazzuca will tell you at the consultation if Forma alone is unlikely to meet your goal.
How many Forma sessions you need
Forma is designed to be repeated. Because each session is gentle, results come from a series rather than a single visit. Typical recommendations:
| Goal | Sessions | Spacing |
|---|---|---|
| Early laxity, prevention | About 4 to 6 | 1 to 2 weeks apart |
| Mild laxity of the jawline and neck | About 6 to 8 | 1 to 2 weeks apart |
| Refresh before an event | 1 to 2 | In the weeks before |
| Maintenance after a series or after Morpheus8 | 1 | Every 1 to 3 months |
These are general ranges and response varies between patients. Your plan is confirmed at your consultation and adjusted as you go.
How much Forma costs in Markham
Forma with Dr. Mazzuca starts at from $350 per session, booked as a 60-minute appointment. As an illustration only, a series of six sessions at the starting price would be from about $2,100. Treating additional areas, or combining Forma with other treatments, is quoted before anything is booked. Because Forma is gentle, many patients see it as an ongoing part of their routine rather than a one-time treatment, so it is worth budgeting for maintenance as well as the initial series.
New patients begin with the $50 Comprehensive Aesthetic Consultation (30 minutes), or a $20 virtual consultation if you are outside the area. Forma is a cosmetic treatment and is not covered by OHIP.
Who Forma is for, and who should wait
Forma suits adults with mild laxity or early loss of firmness who want a gradual, natural improvement with no needles, no numbing and no time off. It is also a good fit for patients who prefer to start with the least invasive option, which is consistent with Dr. Mazzuca's approach, and for patients maintaining the results of Morpheus8 or other treatments. Because radiofrequency does not target pigment, Forma is generally suitable for all skin tones.
Dr. Mazzuca will review your history before recommending it. Treatment is postponed or avoided if you are pregnant or breastfeeding; have a pacemaker, defibrillator or other implanted electronic device; have metal implants in the treatment area; have an active skin infection, cold sore, open wound or rash in the area; have reduced sensation in the area; or have a condition or medication that affects healing or heat tolerance. If you have recently had filler, threads or another procedure in the same area, Dr. Mazzuca will advise on timing. Tell her about any history of skin cancer in the treatment area.
What the appointment is like
- Assessment and plan. Dr. Mazzuca assesses your skin, confirms the areas to treat and the price before starting.
- Preparation. Makeup and jewellery are removed and the skin is cleansed. No numbing is needed.
- Treatment. A layer of gel is applied and the handpiece is glided over each area in slow, continuous movements until the skin reaches the target temperature, which is then held for several minutes. Most patients describe it as a hot stone massage. If any spot feels too hot, say so and the settings or movement are adjusted. Treatment time for the face and neck is typically around 30 to 45 minutes.
- Finish. The gel is removed. You may look flushed for a short time, and you can apply makeup and go straight back to work or errands.
Results: when you see them and how long they last
Many patients notice a slight firmness and a healthy glow straight after a session, partly from warmth and mild swelling, which settles within hours to a day. The lasting change comes from new collagen and builds gradually: most patients notice improved firmness and smoother texture after several sessions, with continued improvement for two to three months after the series is complete. Results are subtle by design; Forma refreshes rather than transforms.
Collagen made in response to treatment is your own tissue, but the natural ageing process continues, so results fade gradually without maintenance. Most patients keep their result with a session every one to three months. Daily sunscreen, not smoking and a good skincare routine help the result last.
Downtime, side effects and risks
There is no real downtime. Expect pink or flushed skin and a feeling of warmth for an hour or a few hours, and occasionally mild swelling or tenderness that settles within a day. Most patients return to work the same day and can wear makeup straight away.
Forma has a good safety profile when performed correctly, and temperature monitoring is built in to reduce the risk of overheating. Uncommon effects include redness or tenderness lasting a day or two and a temporary bumpy or swollen feel. Rarely, radiofrequency treatments can cause small blisters, burns or temporary changes in skin colour. These are why the treatment is performed by a physician who watches the skin throughout and adjusts settings to your response. Contact the clinic if you notice a blister, a burn or redness that does not settle.
Aftercare
- For the rest of the day: avoid extra heat, such as saunas, hot yoga and very hot showers, and strenuous exercise if your skin is still flushed.
- Keep the skin hydrated with a gentle moisturizer and drink water.
- You can usually continue your normal skincare the same evening or next day; pause retinoids or acids for a day or two if your skin feels sensitive.
- Use a broad-spectrum sunscreen SPF 30 or higher every day, and avoid sunburn before and after treatment.
- Let the clinic know before your next session if anything felt too hot or if any area stayed red.
Forma vs Morpheus8, SkinPen and injectables
| Forma | Morpheus8 | SkinPen microneedling | Sculptra & Radiesse | |
|---|---|---|---|---|
| What it does | Radiofrequency heat, no needles | Needles plus radiofrequency heat beneath the surface | Needles only, collagen induction | Injected collagen stimulator |
| Best for | Mild laxity, firmness, maintenance | Laxity, acne scars, texture, firmness | Texture, pores, acne scars, glow | Lost volume and overall support |
| Numbing | Not needed | Topical numbing | Topical numbing | Local anaesthetic in product or cream |
| Downtime | None beyond brief redness | About 2 to 5 days | 1 to 3 days pink | Swelling or bruising for a few days |
| Starting price | $350 | $733 | $350 | $700 |
The short version: Forma is the gentlest way to work on firmness and is ideal if you cannot take time off or want ongoing maintenance. Morpheus8 produces a stronger change in one to three sessions, at the cost of a few days of recovery, and the two are often used together, with Morpheus8 for the main correction and Forma to maintain it. SkinPen is the better choice for texture, pores and scars. Expression lines respond to Botox, not to Forma, and if the jawline and neck are your focus, see neck and jawline rejuvenation.
Why patients choose a physician
At this clinic the person assessing you and the person treating you are the same person, and that person is a licensed physician. Dr. Mazzuca is an MD with CCFP certification in family medicine and additional training in cosmetic medicine, facial anatomy, women's health, and longevity medicine. Treatment is not delegated.
Her stated approach is to emphasise "subtle, elegant results that refresh rather than transform, using the least invasive treatments necessary to achieve safe, natural, and lasting outcomes."
Even a gentle, no-downtime treatment starts with a correct diagnosis. A softening jawline can come from skin laxity, volume loss, bone changes or muscle pull, and only the first responds well to Forma. Dr. Mazzuca assesses which of these is driving your concern, recommends Forma when it is the right tool, and tells you when it is not. She also performs the treatment herself, watching your skin's response throughout rather than leaving it to a technician.

This treatment is from $350 at Dr. Mazzuca's Markham clinic. Exact pricing depends on your assessment and treatment plan, which is confirmed before treatment begins.

Every treatment is performed by Dr. Monique Mazzuca, a licensed physician (MD, CCFP) with additional training in cosmetic medicine and facial anatomy. It is not delegated to a technician.
How long is the appointment?
This appointment is scheduled for 60 minutes, which includes your assessment, the treatment, and aftercare instructions.
Do I need a consultation first?
If you are new to the clinic, book a Comprehensive Aesthetic Consultation ($50, 30 minutes). Dr. Mazzuca assesses your concerns and builds a plan before any treatment is booked.
Where is the clinic located?
The clinic is at 200 Cachet Woods Court, Suite 309, Markham, Ontario L6C 0Z8, in the Cachet Woods area near Woodbine Avenue and Major Mackenzie Drive. Patients travel from Unionville, Richmond Hill, Vaughan, Aurora, Stouffville and across the Greater Toronto Area.
Does Forma hurt?
No, most patients find it relaxing. It feels like a warm, hot stone massage, with occasional moments of stronger heat that pass quickly. No numbing is needed, and the settings are adjusted if any area feels too hot.
Is there any downtime after Forma?
No real downtime. Your skin may be pink and warm for an hour or a few hours. Most patients go straight back to work and can wear makeup immediately.
How many Forma sessions do I need, and what does a course cost?
Most patients have a series of about 4 to 8 sessions spaced one to two weeks apart, then maintenance every one to three months. Each session starts at from $350, so as an illustration a series of six starts from about $2,100. Your plan is confirmed at your consultation.
How long do Forma results last?
Improvement builds over the series and for two to three months afterwards. Because skin keeps ageing, results fade gradually without maintenance, so most patients book a session every one to three months to keep them.
Forma or Morpheus8: which should I choose?
Forma is gentler, needle-free and has no downtime, with subtler results that build over a series. Morpheus8 adds microneedling to the radiofrequency for a stronger change in fewer sessions, with a few days of recovery. Many patients use Morpheus8 for the main correction and Forma to maintain it.
Can I combine Forma with Botox or filler?
Yes. Forma works on skin firmness, Botox softens expression lines and filler restores volume, so they complement each other. Dr. Mazzuca plans the timing, particularly if you have recently had filler in the area being treated.
